Flawless Bridal Makeup Contract

This contract provides the services of Adrian A. Slimp to _______________ . Adrian A. Slimp will provide one make-up application for _____________ on the day of _____________. The recipient, ______________, is obligated to provide a non-refundable deposit of 40% to secure the date. This deposit guarantees and obligates Adrian Slimp to the previously agreed upon date of __________________. This obligation is limited to a one-hour application of make-up per appointment. There will be a surplus charge of $100 per hour beyond ____ hours. This contract is binding and non-negotiable. Adrian Slimp is not responsible for any allergic or negative reactions to the product used during application.
